Action Phase 4: Act
During the fourth and final phase of becoming a Dementia Friendly City we will act together to establish implementation plans for our community goals and identify ways to measure progress.
Step 1: Share the results and involve the community.
We will share the action team’s recommendations and goals with the public and brainstorm actions and programs individuals and organizations can take to help achieve the goals described in the Action Team’s recommendations. These won’t all be government led programs, in fact the vast majority will be efforts by individual leaders in each sector engaged throughout this process. We will then create a comprehensive list of projects and actions individuals hope to take to achieve our goals.
Step 2: Create and implement a Community Action Plan.
Using the recommendations and goals from the Action Team we will draft a comprehensive community action plan that prioritizes opportunities and community priority goals. We will then act together to execute then plan and achieve a shared vision for becoming a dementia friendly community. We will evaluate and celebrate progress and assesse new opportunities along the way to keep the community engaged and informed.
